If it happened within the last 24 hours and the person has not been medically assessed, we recommend urgent medical review today.
If the person is hard to wake, confused, vomiting or getting worse, call 000 now.

Step 5 of 5
This report summarises responses to a concussion screening questionnaire. It supports, but does not replace, assessment by a qualified healthcare professional.
No symptoms reported
Severity bands are MHCG triage guides to help you decide how quickly to seek review. They are not validated diagnostic thresholds.
Recommended next steps
🧠 Relative rest
Reduce activities that clearly worsen symptoms during the first 24–48 hours. Light daily activity is generally encouraged where tolerated.
🏃 Return to activity
Gradually increase cognitive and physical activity. Avoid contact sport or activities with a risk of another impact until assessed and cleared.
👁 Monitor symptoms
Seek further review if symptoms worsen, new symptoms appear or daily activities become difficult.
🏥 Arrange assessment
A clinical assessment should consider the injury, neurological function, balance, eye movements, cognition and exercise tolerance.
🚨 Know the red flags
Call 000 for a severe or worsening headache, vomiting, seizure, increasing confusion, weakness or difficulty waking.
📄 Keep the report
Save or print the full report to share with your GP, school, workplace or sporting club.
Book a concussion assessment
MHCG provides concussion assessments covering the areas above, at our Williamstown clinic. You can also take this report to your GP.
The warning sign selected may indicate a more serious brain or spinal injury.
Call triple zero (000) now and ask for an ambulance, or have someone take you to the nearest emergency department. Do not drive yourself.
On a computer? Use any phone to dial triple zero (000). If you are deaf or have a hearing or speech impairment, call 106 via the National Relay Service. If you're unsure, call anyway — it is always better to be checked.
Concussion screening checklists are only designed for ages 8 and over. A child under 8 who has had a knock to the head should be assessed in person by a health professional.
Call 000 immediately if the child has any of the following: loss of consciousness, vomiting, a seizure, increasing drowsiness or difficulty waking, weakness or numbness, neck pain or tenderness, a severe or worsening headache, increasing confusion or unusual behaviour, or a visible change in head shape.
If none of these apply, arrange a same-day review with your GP, or contact us — we can advise on the right next step for young children.
